Sefton New Directions Home Service has its base in The Investment Centre in Bootle and is made up of the following services: Reablement Care at home Providing a short term service to Sefton residents who have a& reablement potential and are eligible for social care. To appropriately and efficiently reable people, who, upon discharge from hospital have lost their skills for daily living; providing support to re-learn them.& helping people to achieve maximum independence within their own homes, prevent hospital admission or re-admission& & Supported Living Services The service provides& quality person centred support for adults with a learning disability, which promotes independence wellbeing and choice, & that enables people to achieve and sustain a tenancy in ordinary housing in the community, recognising both the rights and the responsibilities that this involves. Supports tenants to actively participate in their community, try new experiences and develop and nurture relationships, building skills in order to live as independently as possible. Shared Lives Shared Living (SL) arrangements offer an alternative and flexible type of accommodation to adults who have been assessed as having a level of need that can be appropriately met through living in an SL setting.& Responsible citizens living in local communities are key to the provision of SL arrangements; People who live in an SL setting actively share in the lives and activities of SL carers and are truly part of the community in which they live. & Woodlands Supported Living & providing a high quality support service to people with severe and/or enduring mental health problems in order to promote independence and wellbeing & Working with housing providers in order to offer people access to good& quality accommodation in the community with tenancies or licences. Providing a reliable and appropriate support service according to assessed needs, listening to the service user and working with them and key professionals. & |
