When you just need a blood test form because of previous medical condition which proved to be quite serious at the time and concluded with hospitalisation , it is my responsibility to ensure that I too ensure I'm monitoring my general help and taking responsibility too. Recently rang to request a form, only to be tole by 2 unhelpful Receptionists that I'd need a phone call- 2 week wait! Or, 3 week wait for 1:1 with Gp to discuss! I just need the Blood Test form for a full-blooded count as I haven't felt myself! Why weren't they able to speak to the doctor and provide the form forme to collect. The system is wrong, if time was taken to take a look at my records and take note of my request then surely this is the right thing to do!!! Who will or who is accountable for not actioning something as simple as looking at my records as a matter of urgency and ensuring Iget the form so can get the care and support I need!

Wow...I need a Blood Test form and prescription review. For a request so simple what a waste of GPS time when he only has to look at mymedical records and provide the form ASAP which I can collect and organise myself! Disheartened, scared worried and frustrated. Whodoypu turn to for advice and support!?!!!
