All seemed very polite but pharmacist kept asking infront of other customers how I was getting on with my medication, this I think should be done privately nit at shop counter. Then he told me to sign a form, I asked what it was and he said a patient review form , told me again to sign it , he had put a cross on form where he wanted me to sign.I picked it up to read it and it was blank, no name ,no address, no medication, no NHS number , nothing blank.I said it was blank he said he would fill it in later.Just sign it.So how would he know who I was later? How would he remember who I was? More importantly having pointed out it was blank he still wanted me to sign it.I refused, I don't sign blank forms and his insistence concerned me.Older people trusting their pharmacist woul sign and not question it.NO ONE should be told to sign a blank form, not asked but told to , I had no choice , don't question him was his attitude.Although pharmacy is usually very good, this is my second contact to with this pharmacist and his attitude leaves a lot to be desired.Who does he think he is telling people to sign blank forms.Should these forms not be regulated? Whoever collects the information do they not want it to be above board?How would that be if people are signing blank forms and someone fills rest out later ,with what information, correct or made up to massage data collected.Completely unsatisfactory.
