Home Care Information |
Derbyshire County Council&s in-house Home Care Service supports people, on a long or short term basis, to remain living in their own home and maintain their independence, including crisis intervention. Support is available 24/7 (depending on assessed need) to people who through frailty, disability, mental ill health or general ill health meet the eligibility criteria. This includes help to carers who are providing substantial personal care and support to partners, relatives or friends. Services may include support with washing, dressing, assisting with meals, assisting with movement (eg. from bed to a chair), with practical tasks (eg. shopping or pension collection), support with finances (eg. paying bills), support for carers (eg.sitting services), assistance with medication, etc, as part of a care package. & Our short term services include re-ablement and dementia services which work with people for a limited time, to help regain skills and confidence to live as independently as possible.& & We offer intensive services for people with longer term conditions or who need end of life care and work closely with partner agencies eg. Health, to ensure we provide quality care. & We have offices around the county providing services local to you, open 7am-6pm and Call Derbyshire who provide 24/7 out-of-hours contact. & We have a robust recruitment process ensuring all necessary checks and training are provided before staff start working with clients. All staff are expected to achieve an appropriate level of care qualification, work flexibly based on service need and within health and safety guidelines. Staff will work to your personal service plan in which you will have specified how you want your care to be delivered. We will provide a weekly schedule notifying you of the visits you will receive.
